PMC – New Client Questionnaire-Postage Preferences

Postage Preferences:

Postage must be received 3 to 5 days prior to a mail date, so please write check payable to “PMC”. 
Do you require a postage invoice? Or is an email or phone call notifying you of the amount adequate?
If this is an on-going mailing, do you want us to set up a Postage Escrow Account and track usage?
If there is excess postage, do you want the funds put into escrow, credited against the invoice amount, or returned as a check?
Will you put money into your own permit number?
Do you have a CAPS account?
Do you have a Business Reply Mail (BRM) account for return mail, or do you need us to open one for you?

Samples:
Our clients like to receive samples for various reasons.  One example of this is to have samples on hand to show their customers what they have to offer.  Samples allow our customers to see what their mailings look like first hand.
What kind of samples would you like: 

  1. Duplicate live names – Do you want us to run duplicate names, i.e., the first 25 records, from the file as samples?
  2. John Q. Samples – Do you want “fake” name/address to be used?
  3. Pull live names out of mailing – Do you want samples pulled out of the live mailing?
  4. Blank inserted samples – Do you want unpersonalized forms inserted into outer envelopes, or blank selfmailers?
  5. No samples required

How many samples do you need? 
Do you want them sent overnight, via courier, or shipped UPS ground?
Some clients want their samples overnighted before the mail drops, while others want them couriered within 24 hours. Others have us send them UPS ground.  How do you prefer to receive them? How fast do you need them?

Personalization:
If we are lasering a letter, can you email us the text of the letter (PDF to show placement, MSWord or text file)?
It would be most helpful to have a PDF of the finished artwork.  This will enable us to provide the closest match to the output you desire.  We would also like to have the text in some type of manipulative format such as MSWord or a richtext file.

Do we laser the signature?  If so, please provide a .tif file or .jpg of the signature.

Is there a specific font or size you want to use?
Please provide as many details regarding the information we will be lasering as possible.  We have a very extensive selection of fonts and sizes but there is always a chance we may not have exactly what you request.  If we know this at the beginning, then we can either obtain the particular font or provide substitute options.
